Print two copies of either, sign them both, mail one copy to the address at the bottom of the form and retain one copy for your records. Enclosing a stamped self addressed return envelope (except by pdf) will help defray costs of operating this registry which is most definitely a non-profit organization.

When you travel in the state, biodiesel vendors can verify through the membership registry that you are a member and have complied with the Department of Measurements Standards requirement that biodiesel users be warned in writing of any potential risk associated with the use of the development engine fuel.
